Lorain, and Medina Counties enrolled in Ohio Department of Health's Children with Medical Handicaps (CMH) program. If your
child has an active Letter of Approval that lists "Nutrition Therapy
Consults" contact us at your earliest convenience to schedule an
appointment. Foodwise, LLC is also an Ohio Medicaid provider.
